SAN DIEGO/PRNewswire/ -- Qualcomm Incorporated (NASDAQ: QCOM) today announced that its subsidiary, Qualcomm Life, Inc., is collaborating with Davita Healthcare Partners and P2Link to develop chronic care management solutions powered by Qualcomm Life's 2net? Device Connectivity Platform and HealthyCircles? Care Coordination Platform to enable continuous care, informed interventions and better management of at-risk populations. The collaborations were announced at Qualcomm Life's annual Connect 2015 ecosystem conference taking place on August 31 and September 1, 2015 in San Diego, California.
Davita HealthCare Partners, one of the largest medical groups and physician network operators, provides integrated care management for approximately 830,000 patients, more than 600,000 of which live in California and represent diverse cultures, socioeconomic groups, ages, and health statuses. The company will integrate Qualcomm Life's 2net Mobile and HealthyCircles technologies into a coordinated care program for patients with heart failure (HF). Patients will be equipped with a home blood pressure monitor, a weight scale and a tablet powered by 2net Mobile to seamlessly collect and transmit the biometric data to the cloud. Patients will also leverage the HealthyCircles Platform to monitor the patient's health status at home, receive relevant educational content and targeted, proactive care management interventions. The initial program will roll out in Q4 2015.

P2Link, a provider of innovative health monitoring solutions, enables payors and providers to reduce costs while improving quality of care for patients with heart failure and chronic obstructive pulmonary disease (COPD). The company is collaborating with Qualcomm Life in the United States and Europe to deploy remote monitoring medical device kits powered by the 2net Hub and Platform for patients suffering from HF and COPD, with the first patient enrollment expected to begin in the United States in Q4 2015. Through their pay-for-performance services that leverage proprietary predictive models and an in-house clinical call center, P2Link targets to reduce the cost of care for HF and COPD patients by 10 to 15 percent, helping payors and providers as they transition to a value-based reimbursement system.

P2Link's first implementation of 2net will be with Emerson Hospital in Concord, Massachusetts.
